  • SELF-REPORTED VACCINES WILL NOT BE HONORED.

    Your vaccination documentation will only be honored if it contains the following details:

    • personal copy (e.g. baby book)or copy of medical chart indicating received vaccines

    • complete date of receipt of vaccination

    • must include signature (license number, if applicable) of doctor or health worker who administered vaccine

  • Medical certificate regarding a previously-treated or currently managed clinically significant illness, Original and 5 photocopies

  • The medical certificate should contain the following information:

    • Medical Treatment

      • name of medication (brand and/or generic name)

      • dosage

      • duration and dates of treatment

    • surgical procedure done

    • complete final diagnosis

    • outcome of treatment or treatment plans and prognosis, if available

  • Physical Examination - Required for all applicants, regardless of age

  • Interferon Gamma Release Assay (IGRA) Test - Required for applicants 2-14 years of age

  • Chest X-ray (CXR) - Required for all adult applicants aged 15 and above, or children who had exposure to TB Cases or history of TB disease

  • Syphilis Test - Required for applicants 18 to 44 years of age

  • Gonorrhea Test - Required for applicants 18 to 24 years of age

COVID-19 Screening and Vaccination
  • Applicants with signs and symptoms of possible COVID 19 will not be allowed to proceed with the medical examination. CDC requires these applicants to be tested for possible COVID-19 by doing an RT PCR test using nasopharyngeal swab and will need to complete the recommended isolation period.

  • All visa applicants who are close contacts of suspected COVID-19 cases will also be required to do RT PCR and complete a quarantine period prior to doing their medical examination.

  • Applicants are asked to bring the original and clear photocopy of their COVID-19 vaccination record.

Important Reminders

  1. All visa applicants 15 years old and above, male or female, are required to have chest x-ray to immigrate. Pregnant women will have proper shielding as a precautionary measure of radiation hazards. Pregnant applicants may have an option to defer medical examination including chest x-ray until delivery.

  2. Minors (17 years old and below) must be accompanied by their parent or by a legal guardian who has thorough knowledge of the child's medical history.

  3. The parent or legal guardian must present a government issued ID (original and 2 photocopies)

    In case the companion is not the legal guardian, the following documents must be presented:

  • Letter of Authorization or Special power of attorney stating that the companion is authorized to sign on behalf of the parent

  • 2 photocopies of the Parent's government issued ID

  • Authorized companion's government issued ID, original and 2 photocopies

  • Applicants are advised to take their meals prior to coming over for their medical examination since food is not allowed within the premises and all the initial blood and urine tests do not require a fasting state.There are restaurants near St. Luke's Medical Center Extension Clinic and drinking fountains are available within the building.

    • Inter-specialty consultations within the clinic (adult cardiology, adult infectious diseases, adult and pediatric pulmonology, adult and pediatric radiology, pediatric allergology, general pediatrics, general surgery, OBGYN, ENT, gastroenterology, oncology, endocrinology, nephrology, hematology, rheumatology and pathology) are performed free of charge. However, psychiatric evaluations shall be charged separately, with expenses to be paid by the applicant.

    • Referrals to sub-specialty clinics outside of St. Luke's Extension Clinic (e.g. neuro-developmental pediatrician, psychologist, dermatologist, interventional cardiologist and radiologist, ophthalmologist, etc.) shall be charged separately, with expenses to be paid by the applicant.

    • St. Luke's Medical Center Extension Clinic accepts cash and credit card payments at the clinic cashier. Applicants who register online may also pay via Security Bank Bills Payment Facility.

    • Medical results and documents will be released according to the physical examination, x-ray and laboratory results, which will be conducted and processed in accordance with the guidelines set by the U.S. Department of State, the U.S. Embassy and the Center for Disease and Control and Prevention.

    • The medical report and visa photographs are submitted by St. Luke's Medical Center Extension Clinic directly to the U.S. Embassy, with few exceptions. In most cases, only the passport, CD of Chest X-ray, and Vaccination Documentation Worksheet are given to the applicants.
